IACUC Application and Protocol Preparation Checklist
Please review this checklist to ensure that your IACUC protocol application is complete before you submit it via the application web interface.
- The Lead Researcher has either a paid UCI faculty appointment greater than or equal to 50% or a Faculty Sponsor.
- All potential co-authors and research personnel have been listed in the protocol along with their role(s) on the project, and each research team member's experience and/or training is clearly described in the protocol narrative. NOTE: Each member of the research team, including the Faculty Sponsor, must:
- Complete the Animal Care and Use Tutorial.
- Enroll in the Lab Animal Occupational Health Program.
- All other applicable approvals have been obtained or are in process (IRB, hSCRO, IBC, Radiation Safety Committee, etc.).
- If research will be funded by an external sponsor (grant, contract, or gift), proposal paperwork has been filed with Sponsored Projects.
- All applicable sections of the IACUC application and Protocol Narrative have been completed, including all required appendices:
- Appendix A: Three-Year Progress Report - required for all renewal applications.
- Appendix B: Create and Maintain a Breeding Colony - required if animals will be bred at UCI for use in the research.
- Appendix C: Hazardous Materials - all investigators should review this form to determine whether it is applicable to their research.
- Appendix D: Use of Prolonged Restraint – required if animals will be physically restrained without sedation or anesthesia.
- Appendix E: Use of Paralytic Agents – required if a neuromuscular blocking agent will be used in live animals.
- Appendix F: Multiple Major Survival Surgery – required if more than one survival (animal will awaken from anesthesia) surgical procedure will be performed on a single animal.
- Appendix G: Production of Antibodies – required if monoclonal or polyclonal antibodies will be produced in live animals.
- Appendix H: Non-UCI Locations – required if animals will be housed or used in procedures at a location other than UCI Irvine, UCIMC or other UCI-owned or –leased space.
- Appendix I: Investigator-Maintained Animals – required if researchers will either provide the husbandry and care for animals housed in ULAR-maintained vivaria OR animals will be housed outside of the vivarium space (e.g., in the lab) for more than 24 hours.
- Appendix J: Request for Housing Exception – required if animals will be held outside of ULAR-managed vivarium space for a period of more than 12 hours.
- Appendix K: Variations in Standard Housing or Husbandry – required for all deviations from the standards of The Guide or standard ULAR-provided care of laboratory animals. Investigators are encouraged to review this form carefully in order to determine whether it is applicable.
